How We Determine Qualifications of Service

 

Military Service

You or your late spouse must have served at least 90 days active duty during wartime and have been honorably discharged. All dates are according to the Veterans Affairs office.

Financial Circumstances

Must have a net worth of less than $155,356. This number excludes your primary residence (under 2 acres) and one vehicle. Any additional acreage is considered an asset. If you are over this amount, we can work with you to find other options in order to qualify.

Assets that do not count towards the total include any items within your home or outdoor equipment. Please contact us if you have further questions regarding assets.

Medical Need

You must require assistance with activities of daily living such as bathing, feeding, dressing. Your doctor will fill out a form to qualify you for this program.

Documents Needed to Enroll

 

Proof of Military Service

A Copy of the veterans DD-214 or discharge papers, and for surviving spouses the Veteran’s Death Certificate is also required. If you don’t have access to these documents, we can assist in obtaining them.

Proof of Income

A Social Security Award Letter, a 1099 for any Pensions, your Last 3 years of Tax return’s (If you file) and a Ratings Letter from VA (if veteran is classified as disabled). Also, if you are still working or receive income through any other source, proof of income will be needed. We would also need the last 3 months of all bank statements to show proof of income, and interest on accounts.

Medical Diagnosis

A VA Form 21-2680 must be completed by a licensed PCP. We will send this to your PCP on your behalf.

Proof of Assets

Complete Bank Account Statements for the last three months. Three most recent statements for all investment accounts (i.e. IRA, CD, Mutual Funds, etc.). Life Insurance Policy Cash Value (if applicable). Documents of government bonds or any other assets not listed above, such as a second home, a second vehicle, if you own more than 2 acres of land, etc.

Proof of Cost of Coverage

A monthly or quarterly statement for either a health care insurance policy or drug plan which you pay for out-of-pocket

For Surviving Spouses

Marriage certificate with Veteran. Marriage certificate for any marriages after the Veteran passed. Death Certificate of the Veteran.
